New Patient Registration
The surgery is undergoing a list capping exercise to ensure we are still able to provide the very best care for our patients. Over the last two years our list size has increased considerably, and we presently have neither the space nor the resources to accept more patients. This is not a permanent situation, and we will let you know via our website when we will be able to next review the application process.

Registration forms are available from reception. On application you will be given an appointment with the Practice nurse for a new patient medical. Overseas Visitors: To be able to register with a GP you must be an ordinary resident of the UK of at least 6 months and be able to prove your residency by providing a property/utility bill in your name, a letter from employer or college/university, a passport or home office documents.
Change of Name, Address or Telephone Number
Please inform us immediately of any change of address or telephone number. There have, unfortunately been a few occasions where the Health Authority has written to a patient at an old address because we have not been informed of the new one, and the letters have been returned to them marked 'not known at this address'. This has resulted in the patient being removed from our patient list. This does not happen often but when it does, can cause distress to the patient concerned.
